- Date of your last day in service (or when you are thinking of leaving).
- Dates of any breaks in service.
- Dates of any previous service.
- If you have a Pension Sharing Order in place as a result of divorce please provide a copy of your ‘Notification of discharge of liability’.
- Annual salary details from 1 Apr 15 to date. These are best presented by way of a screen shot of the JPA self-service salary page which can be found as follows:
- Go to JPA Self Service Employee.
- Select My Information Views.
- Select the Salary tab.
